Petition Letter: Dear Academic Consideration Committee,
I am writing to respectfully request academic consideration for my midterm and final exams due to a combination of physical and mental health challenges. The courses I’m currently enrolled in are:
PSYC 100, Prof Wammes
CHEM 112, Dr.Farnaz
SOCY 100, Dr. Golriz, Ms.Courtemanche
BIO 102, Prof Yates
At the beginning of the academic year at Queen’s, I began experiencing ongoing mental health difficulties and sought support from a therapist. While I did my best to manage these challenges throughout the semester, they became more difficult to cope with toward the end of term. In the days leading up to the final exam, I was also dealing with COVID-related illness, which significantly affected my ability to prepare and perform at my best. Further, I experienced a significant mental health break that resulted in medical support and was diagnosed with depression and anxiety. As the referral to a psychiatrist has been expedited and family doctor and therapist has prescribed an antidepressant that I began on Dec 19 as per the attached prescription receipt.
I will be submitting both a medical doctor’s note and a therapist’s note in support of my request for academic consideration once the holiday closure period has ended and my healthcare providers are able to complete the documentation. The forthcoming doctor’s notes are from:
