Petition Letter: To whom this may concern,

I am writing to formally request a further deferral of my final exam as well as an extension of an incomplete grade for PHAR 370, which was scheduled for January 12 following an initial deferral. I recognize that additional deferrals are granted only under exceptional circumstances, and I do not make this request lightly.

Following the initial deferral, I experienced a significant illness that required hospitalization. Although I was discharged prior to the scheduled deferred examination date, I was instructed by my doctor to take additional time away from academic responsibilities to allow for proper recovery. Despite no longer being hospitalized, I was still experiencing ongoing symptoms during this recovery period.

As a result, I was not fit to write my exam on January 12. In accordance with medical advice, I prioritized recovery to avoid further health complications.

I understand the importance of academic integrity and assessment timelines, and I regret any inconvenience this request may cause. However, the circumstances surrounding my hospitalization and required recovery were beyond my control, and I took this step only after recognizing that I was not capable of completing the examination in a fair or appropriate manner.

I remain fully committed to completing the requirements of PHAR 370. I am submitting this request at the earliest reasonable opportunity and have already communicated an alternate date to write this exam, pending this request being granted.
