Submit an Academic Petition or Appeal

Petition Letter: To whomever it may concern,
In this petition letter, I’m writing to formally communicate the seriousness of an ongoing medical situation that has left me functionally incapacitated for the past week.

For the past week I've experienced daily “episodes” of loss of consciousness. It has become a daily routine of waking up --> feeling extremely dizzy + blurry vision --> loss of depth perception + loss of temperature in my hands and feet, all of which is NOT normal. My body overheats and symptoms worsen. These episodes result in me fainting for various amounts of hours on the floor. For this to be occurring every single day has become a matter of extreme concern for me.

I've visited both urgent care/ER and SWS on campus who have both confirmed something is wrong but are unsure of what “it” is yet. I’ve been advised by both to return to the ER if symptoms persist and to remain on bed rest due to the risk of further episodes. Naturally, I haven't been able to attend my classes/labs this week either but have submitted long-term accommodations with the ArtSci accommodations office.

I have been issued a VEC letter from SWS which I can upload or forward via email if necessary. This has taken such a toll on my body and due to the severity of the symptoms, I am returning home this weekend in an attempt to contact my family/alternate doctor as additional diagnostic resources are required for further medical evaluation which don’t seem to be available in Kingston. My priority at the time is identifying the underlying cause of these episodes and ultimately medical stabilization.

With this being said, I do remain fully committed to completing my academic requirements and am more than happy to write the deferred exam at any time and location once my condition allows. I sincerely apologize for any inconvenience this has caused and appreciate your understanding during this extremely distressing and physically wearying period.
