To the Associate Dean,
I am writing to formally petition for an exception to Academic Regulation 8.4.1 regarding the midterm examination for CISC 151, which I attempted on March 2, 2026. I am requesting that the grade for this assessment (8%) be suppressed and its weight (20%) be redistributed to my final examination.
1. The Nature of the Extenuating Circumstances
From March 2 to March 16, 2026, I suffered from severe, incapacitating symptoms, including a high fever and debilitating headaches. These symptoms rendered me physically and cognitively incapacitated. While I attempted the midterm on March 2, my medical state at the time was such that my judgment was severely impaired. The pressure was so intense that I continually rubbed my head to attempt to stop the pain of the headache which severely affected my ability to write the exam. I was not in a rational position to assess my own fitness to write, nor to understand the long-term academic consequences of attempting the assessment while ill.
2. Evidence of Incapacity and Due Diligence
My inability to seek immediate administrative relief was a direct result of the severity of my illness. I have provided the following documentation to support this timeline:
Medical Documentation: A note from the Queen’s Family Health Team (Consultation: March 23, 2026) confirming the severity and duration of my symptoms.
Communication Logs: Telephone records from March 16, 2026, proving that I contacted the clinic to seek medical intervention at the earliest moment I was physically capable of doing so.
Initial Filing: I previously submitted a request through the standard portal (Reference #76373) as soon as I was able to navigate the system, demonstrating my intent to seek academic consideration.
3. Academic Impact and Request
As a second-year Life Sciences student, I take my academic standing seriously. The grade received on March 2 is a mathematical outlier that resulted solely from a medical crisis and a temporary loss of cognitive clarity regarding my fitness to write. I am fully committed to the learning outcomes of CISC 151 and believe that shifting the weight to the final exam will allow me to be assessed on my true mastery of the material in a fit state.
