Submit an Academic Petition or Appeal

Petition Letter: I am requesting an extension of an exam deferral in these courses; CHEM 112, ANAT 100, BIOL 102 700, BIOL 103 in the Winter term 2026. For reasons that I am registered with QSAS, I am experiencing a severe increase in the symptoms of my condition. This causes me to become mentally overwhelmed, inhibiting me from completing my academic tasks and concentrating on my studies. This has been an ongoing issue for me, however, in the last three to four weeks, my symptoms have reached a severe, and unbearable level.

During my first semester, I experienced similar symptoms, which resulted in challenges to function and achieve success in my academic performance. Heading into the winter semester 2026, I set up and used many resources to help equip myself to make improvements for the second semester, unfortunately, my symptoms have worsened, as mentioned above.

I am working with my resources to determine the best course of action to address my condition and to improve my symptoms. After consulting with my resources, a decision was made to take a 3 month leave, in order to address my health. Despite my current status, I still intend to complete all of my coursework and exams, and am determined to arrange this with my professors. I have attached notes and letters from my health care providers regarding my condition. If additional information is required I can provide as needed, thank you for your consideration.
