To whom it May Concern,

I am appealing to the Faculty of Arts and Science, for Aegrotat status in the three courses for which I was unable to write my final exams last spring - Introduction to Pharmacology (PHARM 100), Brain and Behaviour II (PSYC 370), and Drugs and Alcohol (HLTH 237) - due to extenuating circumstances.

A serious medical incidence on April 6th, 2015, lead to my admittance into Kingston General Hospital, where I was treated by their emergency trauma team, and thereafter counseled by Kate Johnson, and Michael Conrad. Four days later, I was placed in the care of my parents, who then flew me back to Vancouver, where I was admitted into Lions Gate Hospital, then the HOpe Center, for further treatment.

Even though I have been out of the center since May, my bi-weekly medical treatments and counseling, has continued under the care of my psychiatrist, Dr. Philip Severy, and the HOpe Center¿¿s excellent post-trauma team. Although my recovery has been steady, and very encouraging, my doctors, counselors, parents, and myself, have determined that undertaking the stressful task of studying, then writing the three exams referenced above, would not be a wise decision, and could be potentially harmful and detrimental to my progress. Therefore, with my continued health and recovery being my top priority, I am asking the Associate Dean (Studies), Faculty of Arts and Science of Queens University, for Aegrotat status in ¿¿Introduction to Pharmacology¿ (PHARM 100), ¿¿Brain and Behaviour II¿ (PSYC 370), and ¿¿Drugs and Alcohol¿ (HLTH 237). This was not an easy decision to make, and I must thank Dr. Philip Severy, Michael Conrad, and Cindy Butts for their incredible support and positive spirit.

With my new understanding of my illness, its triggers, and how to control its onset, I have recently commenced discussions with my occupational therapist in finding employment opportunities in our community. In time, and with continued improvements in my health, I look forward to finding employment in the kinesiology field, such as becoming an activity coordinator, a personal trainer, or a fitness instructor. Should my Aegrotat status be granted, it would also give me the option to reapply for graduate studies in Physiotherapy, or other Health Care Fields.
