Submit an Academic Petition or Appeal

Petition Letter: Dear Dr. Dorit Naaman,

I am petitioning to drop the following courses: STAM 200 (Summer, 2025), MATH 130 (Fall, 2025), PSYC 271 (Fall, 2025), CHEM 112 (Winter, 2026), PHGY 215 (Winter, 2026), PHGY 216 (Winter, 2026). Additionally, I am requesting credit standing in PSYC 100 (Summer, 2025), BIOL 103 (Fall, 2025), and CISC 101 (Fall, 2025).

I have been experiencing significant mental and physical health challenges since beginning my academic career at Queen’s University, in Fall 2024. I withdrew in Winter 2025 and took a medical leave of absence. From January 2025 to January 2026, I was treated with a medical plan that caused severe adverse side effects. These health issues, compounded by an unexpected personal incident in July 2025, led to severe cognitive and socio-emotional impairments, specifically affecting my ability to concentrate, organize, and process information.

Regarding the Summer and Fall 2025 terms, I was unable to drop my courses by the Week 8 deadlines because the functional impact of my condition and treatment side effects left me in a compromised cognitive state. At that time, I was unable to manage basic self-care or make the administrative decisions necessary to withdraw before the deadlines.

In Winter 2026, I underwent medical assessments in early February and began a new treatment plan. Although I successfully acquired a Verification of Extenuating Circumstances Form from Care Support Services and attempted to follow a recovery plan, my progress was halted by a significant personal family crisis in early March. The stress of this event, combined with ongoing investigations for a chronic health condition, made it impossible for me to complete my academic requirements. The VoE form was withdrawn from the academic considerations website when I registered with QSAS.

Specific course impacts for Winter 2026 include:

- CHEM 112: I remained enrolled past the April 6th deadline because I mistakenly believed I could catch up. Although I completed makeup labs, I was ultimately unable to submit the required lab reports due to my ongoing health investigations.

- PHGY 215: I missed a major assignment on March 13th. After attempting to request an extension, my instructor recommended that I petition for a late drop given my current grade in the course.

- PHGY 216: I missed multiple group assignments and a major project. I sought an extension from my professor but received no response, prompting this request for a late drop.

I have obtained documentation from Kingston General Hospital, Care Support Services, my QSAS portal, and an Off-campus Physician’s Note to verify the timeline and severity of these circumstances. To ensure my future success, I plan to be returning in Fall 2026 with a reduced course load of 9.0 units, and retake the above courses. Should my petition be granted, my cumulative GPA will be raised from its current 0.86 to 1.9, placing me back in good academic standing.
