Petition Letter: First of all, I'd like to thank you for taking the time to read this and your consideration involved in the appeal process. The goal of my appeal is: to request an extension of IN grades to allow me to write my PSYCH 100 and PHARM 100 exam during the December exam period. If this is not possible and there is not enough time to arrange exams (which I am aware is most likely the case at this point), then I wish to write it during the April 2016 exam period.
I was unable to complete my exams during the April 2015 exam period because I was struggling with a mental illness. My symptoms continued through the summer semester and thus I was unable to write my exams at the time either. At one point during this timeframe, I was hospitalized in a Crisis Stabalization Unit and have continued to receive ongoing outpatient psychiatric treatment under the care of my psychiatrist, Dr. Samantha Kelleher. The reason why I was unable to write my exams was because of anxiety and depression symptoms which hindered my education goals. I had a very difficult time focusing on anything school related which included studying for my exams. I have a diagnosis of Post Traumatic Stress Disorder and am a student registered with Disability Services. Both of my course instructors do support my appeal. The supported documentation enclosed is on behalf of my psychiatrist, Dr. Kelleher, who is in Winnipeg, MB.
Finally, I am ready to move forward and write the two exams. I have worked hard to manage my symptoms and have taken the necessary steps towards recovery. This has included finding the right medication combination that works for me, seeing a private therapist as well as having regular sessions with my psychiatrist. It has not been an easy path, but I am determined to succeed and will not let my mental illness stop me from doing so. I am feeling much better and am ready to move forward and write my two exams.
Please consider granting my appeal and allow me to write my exams during the December 2015 or April 2016 exam period.
