Appeal for health issue

My name is Henok Kahsai and a student in Queens University in a continuing distance studies. Due to my recent health Issue I could not focus and perform in my studies.
Earlier in last month of October 2015, I was having severe sharp headache pain which makes me to lose my balance, have difficulties in my vision, limb numbness, poor judgement, speech problem and forgetting things quickly. And because it was getting worse, I had to be admitted in emergency hospital in mid of October 2015. The doctor put me in CT scan and gave me I.V. with different drugs to help me to relief from the pain. Since then, I was following with my family doctor and still I am on medication with Gabapentin 300mg daily. And I have an appointment to see a neurologist for further findings.
The reason why I was late to drop my course is that, I was so determinant and thought that I can handle it no matter what. But, it left me with stress and anxiety and the fact that I forget things quickly make it worse that I could not remember to what I have read in my studies. I believe it is an Extenuating circumstances for me. Not only that, I got almost in couple of accidents in my life to mention one of them is that I was driving crossing and traffic light and for me it was green light, but in reality it was red and I was a couple inches away from getting in collision with the other side driver. So, I am afraid of my life and decided to drop out the course and get medical help.
For further information, I have submitted my roommate's note alongside with the Doctor's note, though the doctor expects a form request to fill out about my situation. I really appreciate in advance for your time to evaluate my circumstances.
