Petition Letter: To whom it may concern

I am 3rd year student-athlete who suffered a concussion during varsity competition in October. This concussion caused me to miss an extended period of time in school. At the time I did communicate with the professor, and received the proper documentation to miss the mid-term in the course. After the concussion symptoms proceeded for several weeks to the point of a make-up assignment being to far gone, the professor recommended and I agreed that we would forego the mid-term and that I should apply for an Aegrotat at the end of the year.

I am applying for this with the full permission and support from my professor. My situation in her eyes and in mine definitely qualified as extenuating circumstances and we both of felt that this was the best route for dealing with the missed mid-term. The professor has also been kind enough to also share with me the mark that I will actually be receiving when this process is complete. If there are any questions about our agreement, the professor will be able to verify, and I have a large email exchange detailing our discussions on this particular matter.

I send this letter to you thanking for you time and understanding in reviewing this matter. I write this letter with full confidence that the academic institution that I attend is of the highest standard and that it has the systems in place to properly deal with my situation. I have no doubt that this will be a quick and easy decision for you.
