Petition Letter: To Whom It May Concern,

I am writing to appeal my final grade that I received in Cognitive Studies (COGS100) in the Fall of 2015. With this appeal, I hope to have the final mark of ¿¿F¿ be removed from my transcript.

On December 11th, I was diagnosed with gastroenteritis and was experiencing symptoms from December 6th- December 14th. I wrote my COGS100 exam while experiencing these symptoms on December 10th. The night before my exam, I sent an email to my Professor to explain the situation. However, I learnt the next morning I had incorrectly typed in her email address and thought it was too late to get any accommodation. Therefore, I wrote the exam feeling nauseated, exhausted, and suffered from a migraine for the full duration and could not concentrate well enough to properly write the exam.

The symptoms I experienced of Gastroenteritis include consistent vomiting and the inability to keep food down. These symptoms negatively affected my sleeping patterns was completely irregular which made me physically and mentally unable to study at the time. The inability to focus on studying put me in jeopardy for all of my exams in December, especially my COGS100 exam.

Also due to this illness, I was also unable to meet my commitments as a member of the Women¿¿s Varsity Volleyball team. To confirm this, I have included a letter from my head coach, Michael Ling.

I have also included a medical note from the doctor at the walk-in clinic I went to on December 11th, 2015 that indicates their diagnosis.

Please consider my application appealing my final COGS100 grade in the hopes to have it removed from my transcript.
