Submit an Academic Petition or Appeal

Petition Letter: To whom it may concern,
I wish to appeal for a late drop for my classes HIST 201, HIST 222, SOCY 276, and SOCY 225 for medical purposes. I have suffered from Social Anxiety Disorder,generalized anxiety, in-attentive type ADHD, and depression for several years and this past semester has been extremely difficult. I have not been able to attend classes due to my anxiety, which has made it difficult to perform well on assignments and tests. My depression has left me with little to no motivation to do school work, my job, leave my house, or perform simple daily tasks.
I was advised by my family doctor, who has been treating me alongside various psychiatrists for years, that I should take the rest of the semester off to focus on my health and therapy. I have increased my dosage for depression and anxiety medications and will be beginning cognitive behavioral therapy in attempt to make my integration back into university smoother and hopefully make my remaining years more successful.
