Dear Dr. Bénard,
Hello, my name is Peter Boos. I am a first year Arts and Sciences student at Queens, student number 10196802. I am writing to you today to request dropping CISC 101 as well as being granted aegrotat standing in SOCY 122B, ECON110B, POLS 110B and PSYC 100B. Since December I have been struggling with various illnesses, which have had me frequently visiting medical walk-in clinics. The week leading up to reading week I was unwell. On February 16th I was seen in the Northumberland Hills Hospital ER and diagnosed with mononucleosis. I was instructed to stay out of school until February 29th. (See attached doctor note 1) Upon my return to school I developed an infection in my throat. I was seen at the Kingston General Hospital ER where I was immediately sent to a throat specialist and prescribed antibiotics as well as given a note excusing me from 2 weeks of school. (See attached doctor note 2) My recovery has been slow. I have missed a lot of course work and am falling behind. I have been in contact with my professors and they are aware of my situation. I expect that in time I will have a full recovery. However, right now my compromised health and resulting lack of attendance may jeopardize my successful completion of this term. I am committed to continuing in this program with a plan to major in Economics. I plan to be back to my full academic potential by the fall semester of 2016.
