To the Associate Dean and to whom it may concern,

I am writing this letter to request that the following courses be dropped from my transcript:
W2012 (ENGL 200) and F/W 2012/2013 (ENGL 271 ; DRAM 300 ; ENGL 200 ; IDIS 210).

This appeal follows the two others I made in 2014 concerning my illness, which gravely affected my academic career for several years while I was studying at Queen¿¿s. I have taken a much needed pause to recuperate since 2014 (close to two years) but am now applying to go back to school in Toronto. Although I was hospitalized throughout the Winter term and into May of 2014, I did not yet understand the extent of my condition prior to that date, which led to its escalation.

I did see several counsellors on campus from 2010 to 2013 but did not consistently seek out help. There were extenuating circumstances as well within my family that had been ongoing for years and these added to my struggles. My own health issues and these problems within my family (such as one of my parent's own mental illness and the other parent's ongoing divorce) oftentimes posed real barriers to my attendance and engagement, and there were many weeks when I did not eat or leave my room. In addition, my cognitive and motor skills were abilities were impaired, and I suffered from invasive thoughts and extreme mood lability. It has been difficult for me to accept that I did so poorly as a student over the past few years and my academics needed to take a back seat to my health in order to facilitate my recovery.

I visited a counsellor at Queen's towards the end of 2013 for one last appointment, but my problems snowballed and I had to be hospitalized in Kingston General Hospital for several weeks a few days after reading week ended. Upon my return to Toronto, I was then hospitalized again for a month and a half at the beginning of April until mid-May 2014. I¿¿ve included a doctor¿¿s note from Dr. Lin who treated me while I was a patient at North York General Hospital in Toronto.

If I could get these courses dropped, now that I have worked hard with my care team towards recovery, it would allow me to continue with University, as my current GPA would be too low to be accepted into another school. My plans are to apply to Ryerson University for English, and York University for English and Theatre. Going to school close to home in Toronto, will allow me to move forward with my studies while continuing to access consistent healthcare from my established support team.

Thank you for your time and consideration,

Samantha Zurin
