Petition Letter
Petition Letter: I am appealing to take 3rd year courses at the University of Ottawa on a Letter of Permission.
I am experiencing ongoing medical difficulties that require me to frequent the Ottawa hospital and other clinics in Ottawa. This is a fairly new diagnosis (fall 2015), and my doctors are still trying to figure out an appropriate treatment plan for me. I am under the care of numerous specialists, most of whom are in Ottawa, and I require ongoing neuroimaging and tests from the Ottawa Hospital.
However, I am still able to do coursework, and would like to continue my studies during this time. For this reason, I would like to take courses for Fall 2016 at the University of Ottawa, so I can also access necessary healthcare in Ottawa.
I have included in my supporting documents 1) support from a medical professional and 2) support from the Undergraduate Chair of psychology in my pursuit of this plan.
I understand that medical difficulties may negatively affect my academic performance at any university. I will respect deadlines placed by the University of Ottawa, and adjust my course load if necessary for my health. I will speak to a disability advisor to arrange accommodations for my school work, as I have done at Queen's, and I will speak to individual professors if I have further needs that arise.
There is little I can do to help my medical condition itself. However, I can try to optimize my health by being with doctors and family in Ottawa, while still pursuing academics. I am consulting with my doctors and am compliant. I still plan to pursue a degree at Queen's, but staying in Ottawa for one semester until my condition is more understood would be the best thing for my health.

Academic Plan
Academic Plan: I am appealing to take courses at another university, so I will make sure that the courses I will be taking there will transfer back to Queen's for a credit. As my health condition progresses and I find out more about next steps in terms of treatment, I will adjust my course load as needed to optimize my health. I will be meeting regularly with medical professionals (General practitioner, neurosurgeon, neurologist, psychiatrist) and a personal counselor. I have included my academic plan in my supporting documents, and my undergraduate chair has reviewed this and approved it.
