Petition Letter

Petition Letter: Dear Associate Dean,

I am writing to appeal for an AG in SOCY 211 and an extended IN grade for SOCY 227.

I was unable to write the final exam for SOCY 211 due to a physical condition that deliberated me from holding a writing utensil. I was admitted to Hotel Dieu's emergency services the night before my exam. I was affected for several weeks after that, the pain repeating intermittently. I have been to multiple clinics and hospitals, but they have sent me home without permanent relief due to the fact that they could not figure out what the cause of the condition was. Although they scheduled me for an MRI, they have not yet contacted me. I have attached records from that day that proves that I was unable to use my left hand, which is my dominant hand.
Although I was granted a make-up exam, I was unable to attend that exam as well due to the same condition and my anxiety disorder. I have already completed 70 of the coursework, for the exam was worth 30.

I have already received permission from Dr. Hand to receive more time to complete the take-home exam for SOCY 227, therefore I would like an extension of the IN grade so I can have more time to complete it.

I have been diagnosed with depression and general anxiety disorder. Having an anxiety disorder prevents me from doing everyday tasks, much less a high-stress assignment such as an exam. My body and mind start to shut down in panic, causing difficulty breathing, concentrating and trembling hands. Although this condition still affects me today, I am currently receiving treatment and finding ways to cope with it. I am taking medication, seeing a therapist on a regular basis and have applied for accommodations through Student Accessibility Services. I am learning to academically succeed through difficult circumstances. I plan to complete the final assignment for SOCY 227 by the end of October.

I will send the appropriate documentations such as hospital records and a note from the doctor I see separately because I do not see my doctor until this upcoming Monday (October 3rd).

Thank you for reading my letter and I hope that you will consider my circumstances. I am appealing for an IN for SOCY 227 and an AG for SOCY 211, and this would help me greatly in reaching my academic goals in the future.
