Dear Sir/Madam,
I am writing to you in hopes that you can appropriately direct my academic appeal to the Commerce Leadership Committee. In early January, I learned that I was not successful in Commerce 172. I met with Gail MacAllister of the Ombudsman¿¿s Office on January 16, 2017 to discuss my personal situation and seek advice on how to proceed with an academic appeal. She explained that I needed documentation from my physician, that I have finally received. As a result, I wish to appeal for a late drop in Commerce 172 based on my medical extenuating circumstances. Attached is a letter from my personal physician Dr. Allison Prokopiak, Ottawa who attests to my medical circumstances during the Fall of 2016. I have provided additional confidential details on the next page in order to provide you with full disclosure of my situation.
