Dear Associate Dean (Studies),
I'm writing to you today to inform you of my temporary withdrawal from my studies at Queens University due to extenuating circumstances. I would like to drop my two current classes ARTH 120B and FRST 290. I would also like to request that my fall 2016 classes be dropped from my transcript, this includes ARTH 120A, CLST 102, CLST 129A, and SPAN 111.
Due to medical reasons, I'm forced to go back to my home town and withdraw from my studies here at Queens University. I have been struggling for several years with severe and disabling mental health issues and this has greatly affected my studies. Finishing assignments and passing exams has been an overwhelming struggle this past year and a half, and due to high stress levels, depression, and anxiety I have been failing my classes. My medical condition has also had a negative impact on my physical health and my family and family doctor are concerned. My family physician, Dr. David Dec, has composed a letter agreeing with my withdrawal (attached) for the time being in order to work on and improve my mental and physical health. This can only be achieved with decreased stress and anxiety levels, medication, and therapy.
My plan is to return to Queens University for the 2018/19 school year, once I have mentally surpassed my struggles and am healthy once again. I plan to continue working closely with my family physician and other professionals to address my mental health so that I can return and finish my studies here at Queens University.
