Petition Letter: To the Associate Dean (Studies),
The Request
I am asking to please have the following F removed from my transcript and grades; Fall 2015 SOCY 389, Fall 2016 GNDS 215 and WRIT 250.


Explanation of Extenuating Circumstances
I have had some serious struggles with mental health over my time at Queen¿¿s. I suffer from panic attacks, suicidal thoughts and other mental health illnesses as outlined in my documentation from my doctor. This documentation also includes a diagnoses of ADHD which has gone undiagnosed for a period of years, until now when I can begin to find tools to help me work against it. These mental illnesses have impaired my studies immensely, however I have recently started working with the accommodations office at Queen¿¿s which has helped me greatly. I have included documentation from this office also, and although it seems to be late in my school career, the results are already showing improvement. I am also working with my family doctor and a psychiatrist who is helping me with my diagnoses and healing. My marks this semester have improved considerably and I am hoping to graduate at the end of this semester. I am on track to do so. By removing the fails off of my transcript it will help me to reach my academic goals, and in turn my career goals. The other documentation I have provided shows when things really came to a head this past summer in August. I had attempted suicide. The following semester I had lots of mental heath struggles which affected my grades resulting in two grades of ¿¿F¿ over the Fall 2016 semester. I also am asking to remove a fail in Fall of 2015, SOCY 389 which was at the start of my severe mental health issue and before I had begun to receive treatment.
