Over the last year, I have experienced significant challenges with my health condition that have negatively impacted my academic performance. As a result of missed classes, I have not been able to completely learn course content, which has impaired my ability to participate in class. In March 2016 I was hospitalized as a result of my poor health. Following this hospitalization, I have been receiving ongoing treatment in the community. However, significant challenges with my health as well as ongoing changes to my treatment plan have made it difficult to participate in classes throughout the Winter 2016 and Winter 2017 school terms.

Completing my undergraduate degree is an important goal for me. In the future, I intended to pursue post-graduate studies. It is necessary for me to do well academically to follow through with my academic and professional plans. Should this appeal be granted, I would remain in good academic standing which will allow me to continue my studies here at Queen's University.

Moving forward, to continue to participate in classes while I manage my condition, I have registered in online courses. These courses allow me to complete the work while managing my health because they have a more flexible schedule. In addition, I have registered with Queen's Student Accessibility Services for accommodations and ongoing support, as needed. My Queen's Student Accessibility Services advisor is named Tess Grant (613.533.6000 X79404). She can provide further information regarding my accommodation plan. I am also continuing to work with my physician to determine an appropriate treatment plan.
