I am writing to appeal for a grade change to my Hist 367 course, as well as (if it remains unresolved by the film department) a previously addressed but non-actuated case of accommodation/grade change for Film 226. This appeal is asserted on the basis of medical based accommodation. Beginning in October of my third year, 2014, I began experiencing significant life stressors following difficult and life threatening mental health cases of two close friends. These cases occurred within two weeks of each other. Following this and seeing my own mental health struggling I sought assistance at LaSalle for guidance at the suggestion of my family doctor. This assistance became as frequent as was possible considering the troubled and delayed nature of organizing appointments at LaSalle. With the guidance of LaSalle I approached my professor for accommodation for a late submission of a final paper in the course which was marked but not resolved on Solus. I had remained in contact with the Professor and later the Department Head Susan Lord and received assurances that the matter would be resolved. Besides this course issue there were others which were resolved at the Professor or Department level. In the wake of this and with the continued decline of my mental health (and physical manifestations following) I sought continued assistance from LaSalle the following year and sought (with the assistance of my GP) private care. This proved fruitless as the practitioners in Kingston had no free spaces. I continued at LaSalle and with my GP for aid. As I began feeling more depressed I fell behind in a number of courses, which I dealt with one by one; the only one now remaining being Hist 367. I have since leaving Kingston at the end of 2016 for home in Ottawa found a private Psychologist who I continue to see in an effort to deal with my mental health and life threatening thoughts. These stressors compounded with heightened anxiety and the need to find work and other more typical stressors have led primarily to the delay of submission to Professor Bailey. I was assured however of his ability and willingness to accommodate my medically based needs. I now stand with all submissions made and I am submitting this form required to complete the changes previously stated.
