Submit an Academic Petition or Appeal

Petition Letter: To whom it may concern,
This letter is for myself, Sarah Tucker, who is currently studying at Queen¿¿s University. I am writing this to put forth my request to waive the appeal deadline for courses, since I was not in a mentally sound position to complete or start the appeal process at the appropriate time.
I have been studying at Queen¿¿s University for the past 2 years, entering with numerous scholarships and the impression that I was on the right path. I was, until April of last year in a mentally abusive relationship that, in combination with poor eating, sleep habits, and the change in workload of first year, left me with severe depression and anxiety. So much so that I found it difficult (and in many cases impossible) to do daily routines and worry about things that at one point would have been extremely important to me (examples being doing well in school, attending classes, taking care of myself, etc.). Such being the case, I was unable to think about the appeal process for my failed classes in first year, as I have just semi-recently overcome the mental boundaries preventing me from seeking help. I have seen a doctor at La Salle for treatment options to heal and stop future episodes, and started seeing a psycho therapist since May 2014 (and plan to continue doing so). I also plan to see Disability Services to check out options for those recovering from depression and anxiety, as well as a learning strategist to make sure I am on track for the 2014-15 year at Queen¿¿s if I am allowed to do so. I have seen a vast improvement within myself since I had sought help, and I feel mentally and physically able to tackle this coming year with safety nets in place so I do not experience what I had in first and second year again.
Attached you will find letters from a doctor I had seen at La Salle during the time of my initial treatment/depression, as well as one from my current psychotherapist. I earnestly want to continue my studies without any disruptions. Therefore, I request you to consider my plea and help me to do so.
