Petition Letter

Petition Letter: Dear Associate Dean,

I am appealing for an Aegrotat standing for CHEM 112 (Fall/Winter, 2016-2017) and for Math 121 (Fall/Winter, 2016-2017).
The reason I am asking for an appeal is because of extenuating circumstances due to a health issue. Due to this I faced several symptoms that severely affected my academic capabilities. The symptoms persisted throughout the second half of the fall term and throughout the entire winter term. The supporting documents provided are verification of illness forms that were provided by the doctor I was seeing in Queens Student Health. The forms provided state the severity of the condition and its effect on my academic ability as well as it gives a timeline. To deal with the issue I was seeing both a family doctor at the wellness center and a specialist at Hotel Dieu Hospital during this time. If required I can get some type of documentation from the specialist if further documentation is required. I am currently seeing another doctor here at my home city to continue treatment. For both these courses I currently have an incomplete currently on my transcript as well with PSYCH 100. I would appeal for PSYCH 100 as well but I was told by an academic advisor it was not allowed in their departments policy. For both MATH 121 and CHEM 112 I have completed 60+ percent of the course content.

For my future plans for academic success I plan on continuing to have regular meetings with medical professionals to help find out a better treatment plan for me.
