Dear Sir or Madam;
I am a third year student in the Faculty of Arts and Science, but have taken the current year off school to deal with ongoing health issues. I am requesting that I may be allowed to submit an Academic Appeal past the deadline for my second year courses. The appeal that I wish to make is to:
- Drop all courses that I failed; and
- Receive Credit standing in the courses that I passed, but earned substantially lower grades.
I make the above request to submit an appeal past the deadline due to the following extenuating circumstances:
- I was extremely sick throughout the school year, and continue to be so now. My entire focus during this time has been on my health and trying to make it through first the school year, and then all of my doctor's appointments; and
- I sent an email to the Academic Appeals Department on July 22, 2017 (before the deadline for the second term) requesting information on how to proceed on this matter. They never replied to my email. On September 24, 2017 I forwarded the original email to the Faculty of Arts and Science and the first response back that I received was on October 16, 2017, and was then advised on October 18, 2017 how to make an appeal. I have attached copies of the email strings.
My illness began in October 2016, when I began to feel extremely fatigued, light headed and unable to concentrate. In the first week of November I had my first fainting episode in which I came close to, but did not, black out. I went to the Urgent Care Centre at Hotel Dieu Hospital, where the fainting episode was diagnosed and they recommended measures to follow to prevent it from happening again. Since the measures they advised were not working, and all of the symptoms continued, at the end of November I went to Toronto to visit my family doctor, who agreed with their diagnosis and recommendation. I therefore had no choice but to return to Kingston, and continue with the measures even though they did not help. In the months following, I continued to feel more and more fatigued, and constantly light headed and several other symptoms appeared. I struggled to get out of bed even after a full night's sleep and it became harder and harder to concentrate. I began to fall behind with my course load as I was always tired and it was very difficult to concentrate on my studies. Just getting through a day was a challenge, and there was no time or energy left in a day to even consider appealing my grades.
Upon returning to Toronto in May, I still did not appeal my grades as my time was spent trying to resolve my medical issues. I was immediately seen by doctors at Medcan, a private health clinic, and was referred to a cardiologist at Toronto General Hospital. I spent the following 2 months undergoing various medical tests, which concluded with a final meeting with my cardiologist on July 13, at which time he concluded that i) the original diagnosis that I was given was not correct and ii) my test results required further investigation by another specialist. It was at this point that there was a break in doctor's appointments for a few weeks, and that I had the time to reach out to Academic Appeals (but did not hear back from them).
I would very much appreciate it if you would allow me to appeal my grades, even though the deadline has passed. My time and energy over the past year has been entirely focused on my health issues, not allowing me the time or energy to even consider making an Academic Appeal.

- Receive Credit standing in the courses that I passed, but earned substantially lower grades (BIOL201, BIOL205, BIOL243 and CLST203).
I make the above request due to extenuating circumstances beyond my control. In October 2016 I began to feel extremely fatigued, light headed and unable to concentrate for any period of time. In the first week of November I had my first presyncope episode, in which I came close to, but did not, black out. I went to the Urgent Care Centre at Hotel Dieu Hospital, where the presyncope was diagnosed and they recommended measures to follow to prevent it from happening again (it was not until the summer of 2017 that I found out that the diagnosis was incorrect). As the measures they advised did not work, and all of the symptoms continued, at the end of November I went to Toronto to visit my family doctor for a second opinion. She agreed with their diagnosis and recommendation. I therefore had no choice but to return to Kingston, and continue with the measures even though they did not help. In the months following, I continued to feel more and more fatigued, constantly light headed and had ongoing presyncope episodes as well as several other symptoms. I struggled to get out of bed even after a full night's sleep and it became harder and harder to concentrate. I began to fall behind with my course load as I was always tired and it was very difficult to concentrate on my studies. I also missed many classes as the fatigue and light headedness made it very difficult to make it to class. Not attending lectures made understanding and staying on top of my work even more difficult.
Since returning to Toronto in May and to this date, I have been seeing doctors at Medcan, a private health clinic, as well as a cardiologist (who performed the tests that showed that my original diagnosis was incorrect), an internist and gastroenterologist, and have referrals for upcoming appointments with a rheumatologist as well as with an environmental health doctor. My symptoms continue to this day, and my illness has yet to be diagnosed.
My plan for achieving academic success depends on the diagnosis that I receive for my illness. I am hoping that I will be diagnosed in the coming weeks/months, and that I will be able to get my symptoms under control. If this is the case, my intention is to re-take all of the courses that I failed, and to get my studies back on track.
