Petition Letter: Dear Associate Dean (Studies),
I am writing to ask that you grant my appeal to apply for a retroactive letter of permission. This letter of permission will allow me to use MUSI 267 from Athabasca University to complete the degree requirements for my BCH (CISC). When completing my letter of permission application last July, I overlooked that a letter of permission granted for summer semester would not be applicable to the course start date available to me, and that it would not be valid after that time. Had I realized, I would have ticked the Fall box when asked to identify the term requested. Although this was an oversight on my part, I would ask that you consider my request as I did try to follow through the appropriate processes and did not apply to take a course elsewhere until I had been granted permission. I have not registered for any other courses since Jan-Apr 2013, since this is my final course for graduation. The table below provides a summary of my actions taken to demonstrate that I was acting in good faith when applying for permission to take the course at Athabasca University.

Academic Plan
Academic Plan: Since this is the final course to complete my CISC-P-BCH degree, I believe an academic plan is not necessary. I achieved a mark of A- in the course, so my academic performance was not in question; rather, I made an administrative error in registering the allow the course from Athabasca to be accepted in my degree at Queen's. I do, however, plan to be more careful in double-checking forms and deadlines in the future.
