Petition Letter: As a person with significant ADHD, skills surrounding organization and executive function have been lifelong struggles. Now in my second year, this past Fall was the first time in my life that I was truly living on my own, and with that came many new responsibilities such as cooking, cleaning, bills, and laundry that were suddenly added on top of an already hefty workload.
In addition to this, I was ill with pneumonia between mid-October and mid-December, which completely crippled my ability to attend class, stay on top of my workload, and meet the many other responsibilities that I already struggled to complete in the first place.
My goal this year has been to apply for an upper-year transfer into Engineering, although now I am very worried about my chances due to my extremely poor performance in MATH 231 and MATH 280 (both are electives - neither are required in order to apply for Engineering). Both courses were highly challenging and attendance in every class was required in order to succeed. As my grades on OnQ will show, my academic performance took a nosedive immediately after I caught pneumonia in mid-October.
I have been assessed by two doctors at Student Wellness Services, and both agree that the combination of my ADHD and pneumonia has had a severe impact on my academic performance.
On a side note, I would like to rewrite the first semester final exams for CHEM 112 and PHYS 104/106 if possible, for the same reasons outlined above.
I hope you will take the things I have said into consideration.

Academic Plan: In regards to pneumonia, I have now made a full recovery. In regards to my learning difficulties relating to ADHD, I have chosen less demanding courses this semester and have begun attending appointments with the Student Academic Success Services, who I believe can help me develop more positive learning strategies going forward. I have also scheduled meetings with my accomodations advisor in the Queen's Student Accessibility Services.
As a current second year student who is applying to enter second-year Engineering next year, I already have more than enough credits than necessary from the combination of first year and second year. As a result, dropping MATH 280 and MATH 231 would not be an issue, and it wouldn't be necessary to make up these lost credits in the future.

New Information: I would like to revoke the request to rewrite the exams for PHYS 104/106 and CHEM 112. My request now is solely for the removal of MATH 280 and MATH 231 from my transcript. I am adding additional documentation regarding my ADHD acommodation as well.
