Dear Associate Dean of Arts & Sciences,

I, Madeleine Bianca Frogley, am putting forth an appeal for an Aegrotat (AG) standing in the course Global Political Economy of Development DEVS 230 (Fall, 2017) on the recommendation of Professor Susanne Soederberg.

In the Fall of 2017, I suffered an extreme health crisis which rendered me unable to complete the scheduled exam for DEVS 230 on 12/08/17 (and also ENGL 200 12/12/17; and ENGL 290 12/20/17 NB: required appeal assignments in progress). On the morning of the 8th of December, Queens Student Wellness Services instructed me to admit myself to Emergency and seek medical attention. Given the severity of the condition, we collectively agreed that it would be prudent to return home immediately and receive care from my personal GP and hospital. I returned home by train and was admitted into Emergency. The attending physician then recommended the required course of action making it impossible for me to return to school in time to complete my remaining scheduled exams

Professor Soederberg recommended shortly after that I could receive an AG for her course because I had completed more then 60 percent of the course work. My physical health was and remains in jeopardy resulting in treatment consisting of multiple weekly appointments including Doctor, nurse practitioner, dietician and Social Worker visits. In addition, I am required to undergo repeated blood work and ECG tests to review status. The team of health professionals monitor any changes and modify treatment as required. It is advised that I remain home in Stouffville for the next 6 months to undergo the prescribed treatment, with parent supervision, to ensure a successful recovery. With the approval of my health team, I currently am enrolled in two online courses recognized in my program and have complete faith I will return in September to continue my full-time studies. In regards to the 2018-19 term and being able to achieve a successful academic comeback, I am working with my family and physicians to ensure there will be no relapse through the following measures:

- Weekly / Bi-Weekly visits with the Queens Student Wellness Services team
- Bi-Weekly Parental Visits to Kingston
- Monthly visits to hometown Physician, dietician and social worker
- Apply for Academic Accommodations to reduce stress and angst when it comes time to exams and or major assignments.
- Change in Program status - current Medial in Global Development and English, changes to a Major in Global Development.
- Complete missed required course Devs 240 in addition to Devs 250, Devs 300, Devs 340, and Itln 101 in the 2018-19 year

I am confident I have the tools to achieve a full and healthy recovery. In addition, my passion for my program and the University remains a strong motivator for return. I would like to thank you in advance for your consideration in this appeal and note that I appreciate this process and the opportunity to put this forth.

Please find attached supporting documentation from my Nurse Practitioner for your records. If you require any additional information, please do not hesitate to contact my health team or me.

Sincerest Regards,

Madeleine Bianca Frogley
