Submit an Academic Petition or Appeal

Petition Letter: I would like to late drop a course, CISC P81, which I took in fall, 2016. I have been suffering from depression and anxiety, and I failed this course due to my social anxiety. We have bi-weekly lab in this course, and the professor advised us to find a partner during the lab. I can not function under the pressure of working with a partner, and I also felt very racially isolated. Then I started to not going to the lab. In the summer of 2017, because I did badly in last school year, I emailed the academic appeal office if I will be put under academic probation, and if it is okay for me to retake the first year courses in next school year. They said I will not due to my mental conditions, and they advised me to late drop my failed course CISC P81. I did not submit an appeal at that time because I stopped seeing my therapist, and I felt awkward asking her for a supporting letter. I started seeing her again in February, 2018, she then took me to Student Wellness Service and got me connected with Dr. Wilkinson, and academic accommodation, which I was too anxious to go on my own. Dr. Wilkinson provided a letter for this appeal, but if more information concerning my mental health in 2016 is needed, my therapist said feel free to contact her at email: info@olmhs.com or phone: 613-532-3777. I would be very much appreciated if this appeal can be processed as soon as possible, dropping this course will allow me to be automatically enrolled in my ideal major. The plan selection will end on June 2, I know I should've submitted it earlier, but I was too anxious to ask the doctor for the letter.
