Submit an Academic Petition or Appeal

Petition Letter: To whom it may concern,


I am Victor Reshetnikov, and this is unfortunately not the first time I am making an appeal to the requirement of withdrawing from Queens. And it pains me to do another one as I hoped this was all in the past.

Last time I appealed I had family circumstances that affected my work ethic and performance. Having to visit home often to tend to family matters that were fueled by my fathers alcohol abuse. Having to work during school to help out family financially, or taking care of my brothers as my mother had more than a handful to deal with - this has obviously affected my work.

Today I am unfortunately writing to you with a completely different explanation that only I was responsible for. Due to the icy conditions just before April, I have managed to slip on my stairs before my house at Queens and hit my head on those stairs on my way back from class. This resulted in an immediate gash that required stitches. At that time I didn't think much of it and had decided to bandage it rather than go to the hospital. Only to find out later that stitches were required, but it was already too late. After about 3 days go by, I still do not feel like myself, and have decided to visit CDK Walk-In Clinic to see if I have by chance received a concussion. To my great surprise I was in-fact concussed. My first instinct was to go back to the Physician and have him fill out and sign the Verification of Personal Health Condition Form, in which he has indicated that my quality of academic work will be affected and I will be fully recovered in 2-4 weeks. This is not my first concussion. But I have made the mistake in seeking a new physician rather than my family doctor who knows my medical history. During the Summer after exams, I still did not feel fully up there, so I have decided to seek out to my Family Physician and see if I still may be concussed. Again to my great surprise I was, and currently still am. My doctor has told me that my condition has worsened and that my anticipated full recovery time now is 8-12 weeks. I am scheduled to get an MRI scan to see the damage, whenever that will be. Attached are both VPHC forms signed by the physicians and my Family Doctor.
