Petition Letter: Dear Academic Board,
I would like to disclose new information to my appeal(4559; submitted on May 14, 2018 in regards to change my grade for MATH121 to a credit standing.
As previously mentioned, I was unable to complete the course to the best of my academic abilities due to family circumstances that impacted my mental health. Upon my arrival at home, I visited my physician and conducted several tests as I was still not feeling well and she concluded that I had been suffering from an acute spell of depression which impacted my nutrition and concentration, these factors then led to me being unable to complete the course requirements to the best of my abilities.
Attached you will find my physician's statement confirming that I was unable to complete my studies to the best of my abilities in the past several months due to medical reasons, if necessary I can attach test results and further evidence to validate this appeal.
