Dear Associate Dean (Studies),

My name is Meaghan Sherban, and I am currently a fourth year student in the Health and Physical Education program. At the time that I entered the program, PACT 100 (Aquatics) was a degree requirement. In the three years since, this has changed and is no longer a degree requirement; however, the grade will remain on my transcript.

During my first year of classes, I participated in the PACT swimming lessons in preparation for my swim test. Two weeks before the test, I sprained my right ankle in a soccer game and have had recurrent issues with it ever since. At this time, I am still unable to complete the swim test, and will not be able to do so for the remainder of the year as I am still undergoing diagnostics and treatment. Since this is my final year here at Queen's, there will not be another opportunity for me to complete this swim test and therefore an 'F' will remain on my transcript.

As I am applying to Teacher's Colleges this December, I ask that a retroactive drop of the PACT 100 course be considered. This would correspond to a change in the grade from 'F' to 'DR'. Since the deadline for applications is December 1st, I ask that you let me know your decision as soon as possible.

Additional medical documentation will be provided that shows I was and currently am unable to complete the swim test requirements.
