Petition Letter: December 4, 2018
Dear Associate Dean of Studies Arts and Science,
I am appealing to drop CLST 130 without academic penalty.
Since arriving at Queen¿¿s University, I have had three occurrences where I had to attend Kingston General Hospital. These were for visits to the emergency room on September 7th, October 13th and October 16th. All of these occasions involved being seen regarding symptoms such as a painful throat, feeling feverish and lethargic, having vertigo and difficulty swallowing and speaking. Subsequently, I was diagnosed with strep throat and Pharyngitis and I was prescribed an antibiotic.
I went home on Wednesday October 24th due to illness and attended a clinic there, where I was prescribed antibiotics on October 28th. The Doctor advised me to be seen again if I did not feel better. I returned to Queen¿¿s the evening of the October 28th.
Moreover, I attended the CDK clinic in Kingston on November 7th and was prescribed antibiotics.
I emailed Dean Crow on November 28th, and on November 30th, I attended the CDK clinic in downtown Kingston. During this visit, I was diagnosed with Exudative Tonsillitis. The Physician advised me that based on previous visits, and my current situation, I would have to prepare for a tonsillectomy in the future. Then, I was prescribed Penicillin.
These mitigating circumstances have caused me to miss classes and I had to ask for academic consideration (which was granted when documents were received) to help me stay on track with my five courses. I have been unable to keep up with my Archaeology class due to illness.
I hope to take a summer course on-line to make up for this credit. I will be attempting to get an appointment with a specialist for this upcoming summer in attempt to have surgery, so that I can prevent any future visits to the emergency department for illnesses. In all, this should help with my overall health and wellness in the near future.

Good evening Dr. Crow,
My name is Mackenzie Stackaruk, and I am a first year Arts and Science student at Queen's University, focusing on a Major in Health Studies. I am requesting to remove CLST 130 (Archaeology) from my timetable as a course withdrawal. I am hoping I could be exempt from any academic penalty due to my mitigating ongoing health circumstances which have influenced this decision.


I have attended the Kingston General Hospital three times this semester, paired with two visitations to other general practitioners at the downtown clinic. Upon these visits, I was given three rounds of strong antibiotics to treat my illnesses. I have been diligently attempting to attend and prevail in my classes and have subsequently completed all my class requirements and class work. However, I have been unable to keep up with my Archaeology subject due to its demands.


I was given the Queen¿¿s academic consideration approval during three situations throughout my illnesses which helped me stay on track with my work. I am genuinely requiring your assistance to grant me this withdrawal without any academic penalty, so my GPA is not affected, which is very crucial for my studies, my program, and I. Moreover, I¿¿d like to share with you that I have battled through Aggressive Idiopathic Scoliosis and I am still persevering through the adverse effects of the spinal fusion surgery I underwent a year ago. Due to my situation, I am hoping you will be able to understand and provide me with this course removal from my transcript.


I sincerely apologize for not being able to withdraw before the deadline; however, I was tremendously ill and had difficulty concentrating on my list of priorities to complete for my classes.
