Petition Letter: Hi there,
I am writing this appeal to hopefully waive the 21-day deadline to appeal for a credit standing and to drop courses that I have not performed well in or failed. Last semester (Fall 2019) I applied for the same appeal because of my struggles with mental health issues. I was granted the appeal in this semester (Winter 2019) because I was just seeking treatment and creating a treatment plan with my physician.
I have been struggling with my mental health all of my second year at university. This is a new problem for me because I have never had issues with my mental health prior to this year. After realizing that I was unwell, I spoke to academic advisors and my physician, which were all accessed through Queen's resources, and worked with them to make a treatment plan that would better my mental health. However, I was still struggling with my mental health in the Winter 2019 semester and was busy making my recovery from mental health issues a priority. Because I was busy trying to better myself, my academic performance suffered. Below, I will detail which courses I am applying for the appeal. I wish to ask for this appeal because I do not want my GPA, which is cumulative over my university career, to suffer from my poor performance due to extenuating circumstances regarding my mental health.
Overall, I was very busy this semester with doctor appointments, new prescriptions, and following through with my treatment plan that my academic performance suffered. I have taken precautions for the future and had conversations with my physician and academic advisor to try and prevent this from happening in the future. Last semester, I provided documentation supporting my claim and would like to use the same documentation as this is the same ongoing issue.

For the course KNPE 255, I received no written or verbal feedback regarding my performance prior to the exam. My exam was worth 100 of my mark and I did not pass the exam.
