Submit an Academic Petition or Appeal

Petition Letter: I am asking for AG standing in ARTH 208.
On April 11th, 2019 I had a term test scheduled at 2:30 pm for the course ARTH 208 worth 30 of my grade. I had studied all weekend for it, and felt very prepared. I had been ill the previous week, but when I woke up on the 11th I felt incredibly sick. I immediately emailed both Professor Reeve and my teaching assistant notifying them of my condition (see attached emails). Both my teaching assistant and Professor Reeve were very understanding, and told me that a take home version of the test would be given to me. As this was close to exam season, and I had three exams in the first week, I waited to finish the three exams before asking for the take home test. It was at this point Professor Reeve told me that I had not submitted anything to the portal.
This was my first time asking for an extension due to an illness, and I had no idea what the procedure was. I went to La Salle walk in clinic the morning of the 11th, where I was seen by a doctor and given a paper copy of the self-declaration - I now know this should have been submitted through the portal - and a verification of my personal health condition signed by a doctor (see attached images). I was under the impression from the secretary at La Salle that handing in the paper copies along with my finished take home test would be sufficient.
This was the third course I had taken with Professor Reeve and I plan on minoring in art history. I thoroughly enjoyed his classes, particularly his assignments, and I'm disappointed that I missed this. Due to missing the test, my grade in the class dropped to a D+ which is a much lower mark than I expected to receive in this class. This was a total case of miscommunication and misunderstanding, and I hope this appeal can reinstate the grade I had going into the last test.
