Sub: Request for Incomplete Grade not to appear on my official transcript
To Whomsoever It May Concern
From the beginning of the summer term, I have been depressed due to a sexual assault that occurred in May of this year. I submitted a Self-Declaration of Brief Absence
request to both of my Professors on May 24th and thought that I might be able to proceed in my summer studies. I found myself struggling and falling behind in my studies. After my assault, I found myself with post-traumatic stress disorder (PTSD), including flashbacks, nightmares, and anxiety. I have been experiencing depression, including prolonged sadness, feelings of hopelessness, weight loss, and loss of energy. I have found that I have not been able to focus on work or schoolwork, as well as not feeling present in everyday situations. I
am a single parent of three children, and I found myself struggling to keep ahead of my studies.
I did not submit any extenuating circumstances letters during the term as I was in denial of my situation and embarrassed to tell anybody and reach out for help. After speaking to my doctor, I decided to reach out to the Academic Advisors to see what I could do.
With the help of resources from my Academic Advisor and my Doctor, I have been seeking treatment for my physical and mental injuries. My recovery has included mental health counselling and attending a support group and medication to help with my depression.
I am a good student, as you can see from my grades from the prior academic term, and I intend to get back on track as my education is of prime importance to me. I just began my next summer semester course, and I believe with the support that I am receiving, I can continue my studies and be successful in them. I have requested and have been approved to take a vacation from work for the month of August to focus on school and my recovery.
Therefore, due to my extenuating circumstance, I am requesting that I be able to withdraw from SPAN111 and ENGL100 and not have it reflected on my official transcripts. I have attached a letter from my doctor requesting a leave from school from May 9 ¿¿ July 31, 2019.

Academic Plan: As stated in my letter, my recovery has included mental health counselling and attending a support group and medication to help with my depression.
I am a good student, as you can see from my grades from the prior academic term, and I intend to get back on track as my education is of prime importance to me. I just began my next summer semester course, and I believe with the support that I am receiving, I can continue my studies and be successful in them. I have requested and have been approved to take a vacation from work for the month of August to focus on school and my recovery.
I have registered for a full-time course load for the Fall and Winter. I will use the following tools to help me be successful in my studies.
I will make sure I stay organized and set a schedule by making a plan for what I am going to do and when I am going to do it will make sure that I am always ahead of the curve.
I will divide it up up my task into manageable chunks and give myself enough time to complete and seek help if I need to from my Professors and TAs.
Make sure that I get eight hours of sleep every night.
Not be afraid to ask questions and advise and use the resources offered by academic advisors.
Have regular check-ins with my doctor and support group and continue my therapy and medication to support good mental health practices and behaviours.
