Petition Letter: The request
Dear Associate Dean Studies,
I am requesting an extension of the Deadline to Submit Incomplete Work in PHIL 296 (Winter 2019).
I am requesting aegrogat standing in PHAR 100 (Winter 2019).
And I am requesting to have FREN 219 (Fall 2018), CHEM 211 (Fall 2018), CHEM 212 (Fall 2018), CHEM 213 (Fall 2018), CHEM 221 (Winter 2019), and MATH 112 (Winter 2019) dropped off of my transcript.

Explanation of Extenuating Circumstances
For several years I dealt with undiagnosed depression and anxiety and managed to get through my schooling. As I got further into my university career the courses became more demanding and I was unable to maintain my standing in school due to my mental health. As my mental health was left unattended to, my performance in school declined which in turn caused me to have negative thoughts towards school. These negative thoughts in turn caused my school performance to decline, continuing an unhealthy cycle.
The winter semester of 2018 is when I began to notice my mental health affecting my schooling however it wasn¿¿t until the following school year in the fall semester of 2018 that I finally sought help for my condition. At the end of October 2018, I was diagnosed with major depressive disorder and generalized anxiety disorder and prescribed medication to help me handle these conditions. Since recovery and balancing moods is different in everyone, it took a few months for the proper medication and dosages to be prescribed and to know if they were helping me. In January of 2019, I started seeing a therapist in addition to taking the medication in hopes that it would further aid in my progress. I believe it did, after being on the medication and consistently seeing a therapist for several months I can say that my mood and mental health has stabilized at a more positive level. I think it is visible in my marks from the winter semester that I was making some sort of progress as I was able to pass more of my courses.
Currently I am enrolled in the chemistry major however, my intention is to switch my major to health studies after the upcoming school year. I am appealing to have CHEM 211, CHEM 212, CHEM 213, and CHEM 221 dropped off of my transcript as they will not apply to the major I am hoping to be in and they are not a reflection of my academic ability
The three chemistry courses I was taking in the fall semester as well as the chemistry course I was taking in the winter semester were very demanding and required a lot of attention. I was focusing the majority of the energy that I did have on my depressive thoughts and anxiety as opposed to my schoolwork, causing me to be unable to put the proper effort into the courses and come out of them with passing grades. Unfortunately, courses very heavy in math are a trigger for me, and any time spent on these courses would further encourage the cycle of depression, causing me to spend less and less time and mental effort on them as I got deeper into the semester. In the winter semester of 2019, I was still on the path to major in chemistry and in the mindset that I wanted to major in chemistry and as such, I was taking MATH 112, another course that I would have difficulty focusing my energy on. As I am no longer trying to stay on the chemistry path, I no longer require any of these courses and don¿¿t believe they are a true reflection of my academic ability.
I am appealing to have FREN 219 dropped off of my transcript as it is not a representation of my French abilities, as shown by my passing mark in FREN 150 the previous year. In the fall semester of 2018 I was so consumed by my depression and poor performance in my other courses that I didn¿¿t allot enough time to successfully complete the requirements of FREN 219. This included skipping the midterm and final exams even though I had prepared as I had no confidence in my academic ability and saw no point in writing evaluations which I would not do well in.
I am appealing for an extension of the deadline to submit incomplete work for PHIL 296 as I am enrolled in a philosophy minor and hope to apply the credit to that. The work I didn¿¿t complete was the final essay, even after spending large amounts of time on it. I was overwhelmed by my final exams which I knew I would not do well in due to my academic performance throughout the term and unable to properly apply myself to write an essay. Because the essay is on a topic I am passionate about, every time I worked on it my passion for the topic brought up a lot of anxiety as I worried that I would not do the topic justice or get my point across properly as I had set such high standards for myself. I believe that if I am able to complete and submit the work now it will be a true reflection of my abilities and I will be able to submit something I am proud of as I am now in a better place mentally.
I am requesting aegrotat standing in PHAR 100 as my depression and anxiety hindered me from completing all of the course work however I received good marks on the work that I did complete, which I believe is a true representation of my understanding in the course.
