¿¿ Tuition fee refers to a tuition, course, term, program, audit or tuition deposit fee.
¿¿ A fee amendment refers to reassessing or refunding a tuition fee or an ancillary fee that has been assessed by the Office of the University Registrar (OUR). This normally occurs as a result of a change of registration.
Responsibilities and Authority
¿¿ Faculties and Schools have the responsibility and authority to approve course registrations; and to approve course registration changes.
¿¿ The Vice-Principal (Academic) has the responsibility and authority for the assessment of tuition, ancillary and administrative fees. The authority for assessment of these fees is delegated to the University Registrar.
Please note: Any change in academic registration and/or assessed tuition fees may impact government student financial assistance eligibility and/or funding. Please consult with the Student Awards Office, Gordon Hall Room 125, Office of the University Registrar, to understand the potential implications. It may also affect T2202A¿¿s. It is the responsibility of the student to request an amended T2202A.
The effective date of a course registration approved by a student¿¿s Faculty or School will normally coincide with the date the student initiates the change request. In exceptional cases, where there are documented extenuating circumstances, an appeal may be granted to amend a portion of the related tuition fee outside of the published full tuition refund schedule.
It is the sole responsibility of students to ensure their course schedule is accurate. Tuition fee amendments will not be accepted for requests related to the student¿¿s failure to ensure that a class is properly dropped or added on SOLUS.

The following outlines the circumstances under which requests for fee amendments for undergraduate students may be approved:
¿¿¿The student did not receive the academic service or delivery for which the fee was assessed. This may result from:
¿¿Something out of the student¿¿s control occurs during a term and this prevents the student from attending and completing studies during the term, after the published course change and refund deadline, and there is evidence (e.g., written documentation) that the student was not able to attend class. For example, cases where family emergencies or health reasons force a student to withdraw during an academic session.
¿¿Written documentation must be included with appeals. Examples of written documentation include, but are not limited to:
¿¿A letter from a health care provider outlining circumstances
¿¿A note from the instructor of each class being appealed, and detailing
the last date of class attendance / participation
It remains the responsibility of students to familiarize themselves with fee policies, procedures, deadlines and rates.

Reason for Appeal: Briefly explain the extenuating circumstances and include supporting documentation with this appeal form.
I was prescribed an antidepressant medication on the 18th of September that I would be titrated up to higher dosages in the following two weeks. By the 7th of October I was experiencing an extreme adverse reaction to the medication that then led to a complete inability to participate in academic activities. I was experiencing panic attacks several times a daily, was constantly nauseous and trembling, and I was terrified resulting in unrelenting emotional distress. My sleep and eating habits were tremendously disrupted and I sought medical attention on the 15th of October where I was placed on verification of illness at the utmost severity until the 10th of December. I made an appointment with an academic advisor, Spring Forsberg with the Faculty of Arts and Science who then immediately removed me from BIO 243 and then CHEM 281 by the 10th of October. I am still recovering from this horrible adverse reaction to that medication and the prerecessions have been detrimental to my health. At the time, I was fully unable to complete any course work and was assessed by Dr. Butler and Dr. McNevin at Student Wellness as well as I met with Natasha Degroot who can attest to my condition at the time.
Moving forward with my education, I have sought out extensive medical care however, my family is not understanding of my present condition due to partly that reaction and their extensive abuse over many years- leading me to have high anxiety and panic attacks. After that fall I have tried to get my degree back on trac by. enrolling in these summer courses. owner, after returning home for a family funeral, only to be further physically abused and verbally attacked for several weeks where I could not get a moment to study. I reached out to my therapist at the time, and even didn't attend my own brothers wedding as a result on the tremendously abusive time I had right in the middle of the start of lots of course work. I was physically abused while sitting down attempting to stay late one night and was advised by friends to drop the courses, however it was past the due date at that point. I am now seeking to focus on my studies by removing myself from my family, however with that new financial burden, the possibility of having that tuition refund for which I would otherwise have dropped the courses by the drop date, would be so kindly appreciated. Thank you.
