Submit an Academic Petition or Appeal

New Information: For why I didn't do this on Sep 19th, it's because I went to the undergraduate office and told to see the chairman of undergraduate students, which is Ian Cromb. At that point, I don't know when his office is open. It was until yesterday I got to talk with him and knew that I need to sent an appeal.
