Petition Letter: To whom it may concern,
I was hoping to apply to receive course credit instead of a grade for one the courses I took in Winter semester last year. I recently came to understand that I was supposed to have done this by September 15th of this semester. I was unable to apply earlier due to trouble getting together the necessary documentation from my psychiatrist. Though I made the request for documentation early in the summer she was out of the country for a large part of that time and I had my first appointment with her today which was the first time she was able to provide me with signed documentation.
I was hoping that that, and the ongoing nature of my disabilities, bipolar disorder for which I'm being treated by my psychiatrist, but have yet to find a medication regime that satisfactorily controls my symptoms, and my severe anxiety and ADHD for which I am registered with QSAS, but which I cannot be medicated for due to my bipolar disorder, might excuse delayed application for appeal.

New Information: I'm appealing for course credit status for chem 321 in winter 2019.
Essentially I'm appealing because my rapid cycling type 2 bipolar disorder which I was finally diagnosed with last fall (and had 6 weeks of severe accommodations for) severely impacted my school performance last semester. I went through phases of not being able to leave my home, as well as manic periods in which my thoughts were confused and I was not at all able to function properly.
The medication for it needs to be upped slowly so was not at all affective throughout that entire period.
All my courses were severely affected though I was able to maintain not severely bad grades in the other three due to a combination of professors giving me extremely flexible assignment hand in dates, group work and fortunate timing for midterms. CHEM 321 was not structured to allow that and also was taught in a fashion that required extremely good classroom attendance. I had difficulty advocating for myself and accessing resources throughout this period.
I've attached a copy of the letter she'd originally prepared for me which I hadn't been able to access until my appointment with her in early November.
