Petition Letter

Petition Letter: To whom it may concern;
My name is Rachel Korchinsky (20061156). I am requesting to CR my Quantum Mechanics course CHEM 313. On my visit to my Optometrist over reading week (Oct 24/2019 ¿¿ Oct 27/2019), I was diagnosed with an additional, different eye condition as stated in the medical documentation provided. In addition, the new accommodations were to have been in place by mid to late November 2019 as I met with QUSAS on November 18th 2019.
I am currently being accommodated for the following four diagnoses regarding my eyes which are as follows;
1) Accommodative infacility
2) Fusional vergence dysfunction
3) Vergence infacility
4) Eye movement/ tracking difficulties.
However, with this completely new medical issue, which is unrelated to the other four, I required further changes to my accommodations and was not being accommodated in the correct ways for my medical issue I was unaware of.
This affects CHEM 313, as it involves intense mathematical equations and problems which are affected as I shift my numbers. This is different than my other medical issues as clarified in my medical documentation.
With this information presented and as it pertains to a new and different diagnoses than the ones to which I am currently being accommodated for, I am requesting a CR in my CHEM 313. As this was a medical issue, I was unaware of and undiagnosed until half of my semester was completed. This was after midterms/tests and assignments were completed.
In addition to this information I will be completing this course without neuro visual training as I will not be able to go for treatment to help my situation until December 2019 as the clinic is in Toronto and not Kingston where my place of study is located. On a side note, I also did not receive my new prism prescription with an increase in strength for my glasses until the middle of November, after my Oct 2019 diagnosis.
