Petition Letter: To whom it may concern:

I am writing to you today to request that GPHY 314 be dropped from my transcript, and to apply for CREDIT standing in PSYC 370. I am requesting this because I experienced extenuating circumstances with my health during the second term of the 2018-19 school year. My illness is a chronic condition that is episodic in nature. I started off the year feeling very strong and in good standing, However, my health worsened over the course of the winter semester because of my inability to obtain treatment as I awaited a surgery that was scheduled in April. I unfortunately was forced to miss a lot of class time as I was in and out of doctor¿¿s appointments as well as the Emergency Room here in Kingston.
In regard to GPHY 314, when I fell ill, I had not yet completed 50 of the course work, previously feeling that I had ample time during the balance of the term to catch up/recover for my courses. However, the episode I experienced occurred after the drop deadline, which set me further behind. My academic advisor encouraged me to pursue an appeal for GPHY in order to work on completing my other classes in the meantime. I managed to finish the rest of the semester by deferring all of my exams to the summer because of the severity of my condition. I spoke to Dr. Menard, my instructor for PSYC 370 who encouraged me to consider applying for credit standing in her course because of the vast amount of class time I had missed. This course is very reliant on lecture material on complex subjects, and thus my time away did a great disservice to my ability to be successful in the course. As I am registered with QSAS, I did my best to continue on in the course, doing the very best I could given that I was missing so much class time. Therefore, after discussions with my academic advisor and Dr. Menard, I am requesting CREDIT standing in this class as I do not feel it is representative of my abilities had I had the opportunity to be present.
The severity of my illness during that time, which is corroborated in the letter from my doctor, impacted my studies as I was not present in class. Having the deferral of my other 3 exams allowed me to focus on finishing the summer and fall term strongly without having to drop more courses. You can see from my grades now, that although my health is variable - it is improving. I was able to obtain the surgery I needed and am now receiving treatment regularly and am succeeding in my current coursework.
The office should have my submitted medical documentation on file, as well as the documentation that I have registered from QSAS. If necessary, I am happy to supply further information.
