Petition Letter

Petition Letter: Prior to my second midterm exam I had emailed my instructor my letter of accommodations from from Queens on two separate occasions, once at the beginning of the term and a second time prior to the second midterm. Upon reviewing my letter of accommodations the professor emailed me back saying the 2nd midterm is scheduled for 7pm, hope thats okay, however my accommodations specifically state no night exams. For the actual exam, the proctor showed up 5 to 10 minutes late, there was only one proctor for two rooms and she did not tell the students which room to go into and ran off to do something after opening the doors. I entered the exam room and sat for another few minutes waiting for her to return. Once she returned I asked her which room I was supposed to be in as my accommodations are to take exams in a private room, not with other students. she informed me that I was supposed to be in the other exam room and that she had already started their exam. The proctor gave me my exam and told me to get my stuff and walk down the hall to the other room and find a seat as well as to be quite because they had already started. she did not accompany me to the other room. Once I got to the other room and found a seat amongst the other students (again not a private room) nor was a computer or reader provided (also in my accommodations). I assumed I should start the exam which I did. About 20 minutes into starting the exam the proctor came back to the room and said she had to see my letter of accommodations. I had to stop taking the exam, retrieve my phone and search through my email for the PDF of the letter I had sent my professor at which point the proctor squinted at my phone screen for a few moments before thanking me, all while I was sitting at the desk with my exam. Towards the end of the exam the proctor made an announcement that once finished students should bring their exams over to the other room where she would be. I wrote multiple times on the exam that I could not see various aspects as a computer with zoom was not provided. The extenuating circumstance lasted for the duration of the exam which was made difficult to complete and focus on due to an improper testing environment. The exam in question composed at least 25 of the final grade in the course.
