Petition Letter
Petition Letter: I am appealing for Credit (CR) Standing in CISC 220 (C-) and CISC 282 (D+) as a result of health- related extenuating circumstances.
My health condition is new this year. During Fall Term 2019 I tried initially to self-manage my symptoms. However, many days I was unable to get sufficient sleep/any sleep, had difficulty with concentration and memory due to my extreme fatigue. This significantly affected my ability to attend classes and complete assignments/tests. As such, I do not feel that my course marks during this term are reflective of my true ability.
TIMELINE:
¿¿ Intermittent at the beginning of Fall Term 2019. While I was still able to attend classes and submit/attempt most assignments and tests, my marks started to decline because I was constantly fatigued and had difficulty concentrating. I was unable to complete tests in the time period assigned and started making mistakes e.g., missing questions on assignments.
¿¿ At the later part of the term, I started not being able to sleep at all 3-4 nights per week. During this period my sleep patterns were unpredictable resulting in missed classes, assignments and tests. I was unable to function most days due to extreme fatigue. I was unable to concentrate to study and retain information. I had difficulty making decisions e.g., what to put on an equation sheet for a test.
¿¿ Sought health care on two occasions from Student Wellness Services and from a doctor in the community, and obtained counseling during the Fall exam period (December 2019). Short-term medication provided for exams. This left me groggy during the day.
¿¿ My symptoms became severe in the Winter Term and I had to drop 4 of my 5 courses because of complications with medication/delayed treatment.
NEXT STEPS: I continue to work with Student Wellness Services (see attached to support). I have also been seeing a Psychotherapist in the Community on a bi-weekly basis. I plan to work with QSAS on ongoing accommodations and continue with counseling. I further plan to continue working with Dr. Hamilton at Student Wellness Services on a treatment plan that will allow me to succeed next year.
This has been an extremely difficult school year. My health condition resulted in both mental and physical health consequences for which I am seeking ongoing support from Student Wellness Services. I hope that you will accept my appeal for Credit Standing so that my poor academic performance in Fall 2019 does not impact any future academic opportunities that I may wish to pursue that are GPA dependent. Thank you for your consideration.
