Dear: Associate Dean (Academic)

I am formally requesting to withdraw and defer until further notice from LAW 201/701. Due to unforeseen medical circumstance, I am unable to concentrate on my studies. As an interest student, I am hoping to be able to request a credit on my Queens account with the intent to re-enroll, and continue my course when physically able.

I am only withdrawing from this one course as this is all I am currently enrolled in due to being an interest student.

I am currently being treated for a serious spinal injury. I have had spinal surgery in the past and have re-injured my back, however unable to figure what the cause of the injury was. This injury occurred last week and I am now under my physicians care. I have an emergency MRI booked for July 8 with the assumption I may need to have another surgery.

With the medication that I am on, it does cause side effects, such as dizziness, drowsiness as well as others. Unfortunately, I am in a great deal of physical pain that has taken over much of my daily life.

I am unable to stand, sit, or concentrate for even short periods of time.

I had considered asking for an extension for the work, however due to my current mental condition in regards to the pain I am experiencing and the side effects of the medication, I do not feel I would be able to put forth my best work and by advice from my physician to not do anything beyond my daily living.

I am hoping to be able to re-start in January 2021 as if I do have surgery it will allow for adequate time to adjust to my new circumstance. I am wishing to complete the four courses of sturdy towards my Certificate in Law.

Please advise of what further steps I may need to take to formally ask for a financial credit on my Queens account to use at a later date.
