Re: Samantha Craig, Student 20157440 ¿¿ Request to have all course marks, to date, expunged
I am appealing to have all of my first year courses expunged from record.
I have experienced extenuating circumstances this past school year that I am still in process of dealing with under doctors¿¿ care. I have depression and anxiety, and it seemed that the medication I was on was not the right one, as I was struggling. To deal with this, while at Queen¿¿s, I was seeing a psychologist on campus; although somewhat helpful, not as effective as required. On February 27, 2020, I fell and suffered a severe concussion. I went to KGH and had a CT scan and a concussion was diagnosed. The concussion has added a number of complications (altered eye sight, inability to focus, severe headaches, lethargy, and so on), and has compounded my depression and anxiety levels.
Forms from both my GP, Dr. Tolkin, who has been monitoring me regularly since the concussion and from the Psychiatist, Dr. Tugg, who is working with me to find the right medication for my depression and anxiety, are appended. Through the Concussion Network in Toronto, I was connected with Dr. Loh, of Toronto Rehab, who has referred me to a Neuropsychiatrist ¿¿ who I will meet with for the first time in December, 2020.
I have been unable until now to face contacting Queen¿¿s to make this request to expunge my courses from record, for which I apologize. With assistance from my mother, this letter is being written.
It is my hope that I might be able to apply for one course this coming school year, with a goal of returning full time to Queen¿¿s next September, 2021. My doctors and family support this decision, as I still require time to continue to heal and to deal more effectively with my depression and anxiety.
