Petition Letter

Petition Letter: I am writing to the Associate Dean of the Faculty of Arts and Science to waive the 21 day deadline to submit an appeal. The late appeal is regarding late withdraw of courses or removal of courses from my transcript, due to extenuating medical circumstances.

I became ill in November 2011, which impacted my ability to focus on Academics. I saw a health care professional at LaSalle, during the time of physical illness (note enclosed). These out of control circumstances have directly affected my academic performance in multiple courses, specifically: CLST330, CLST303, CLST340, CLST306, CLST205 and HIST218. I experienced psychological challenges during and post physical illness. To address deteriorating mental health issues, I sought help from Heath Counselling and Disability services, the HCDS (letter enclosed). Throughout the following years, it became apparent I could not handle the post-effects of my illness and my academic responsibilities. Subsequently, I was required to withdraw from Queens for a period of 1 year. Upon my return to Queens it has come to my attention my need to write an appeal to waive the appeal deadline.

If this deadline appeal is granted it would allow me to submit the appeal to get the aforementioned courses removed from my transcript. This would allow me to be able to continue my education. Since my return to Queens, I have raised my GPA and managed to be successful in course management and completion. With continued academic success I plan to be a Queens graduate in 2015. Please contact me should you require further information or have any unanswered questions.

Thank you for your attention.
