Submit an Academic Petition or Appeal

Petition Letter: I am writing this letter to request to postpone my studies for this semester due to my severe health condition. I'm suffering from severe physical illness (sciatic nerve problem). My situation has deteriorated recently. My condition makes extremely difficult to sit, walk and pend and I am in severe pain all the time.
I have enclosed the medical report from my doctor ( Hospital of Ottawa) that further explains my illness
I am scheduled to have a surgery within six weeks and will need to undergo physiotherapy after that. Taken into account my current circumstances, I would highly appreciate if you could consider my request to defer my study for this semester.
