Petition Letter: Hello,
My name is Adam Collier and I'm a 4th-year Film (Ba) student. My student number is 20122572.
I am appealing to re-enroll in the Film 217 class.
I was recently diagnosed with ADHD, and have been struggling to concentrate on finishing assignments throughout my time at university. My concentration and time management abilities hinder my ability to study and meet academic deadlines. Since receiving my diagnosis I have tried medication and counseling with a healthcare professional. However, I'm still struggling to find a method/structure that helps me.
This year I did not hand in my midterm for Film 217, despite working on the assignment for multiple days. I started a dialogue with my teaching assistant and professor, asking if would be able to hand in what I had written for the midterm. Their response was that I could write an alternative midterm essay. Unfortunately, I had dropped the course to avoid the academic deadline, thinking that I would receive a zero for this evaluation.
I would like to continue the course so that I can complete the alternative midterm evaluation, and work towards a good mark in the class.

Academic Plan: To succeed in my future studies I will take multiple steps to ensure that I improve my study habits and mental wellness to reach my academic goals. I will do this by taking a reduced my course load, making appointments with Student Wellness Services, and consulting with a learning strategist in Student Academic Services. I will also make personal adjustments to change my daily routine by avoiding distracting environments and improving my personal health. My main goal is to complete a Bachelor's degree in film and to accomplish this I need to repair my GPA, re-taking classes that I have failed.
