Petition Letter: To whom it may concern:
Due to an escalating health crisis, I am seeking to late withdraw from one of my subjects in an effort to salvage the other courses. I suffer from moderate to severe clinical generalized anxiety disorder, have a medical diagnosis and am registered with QSAS. I have completed many treatment programs. I started having panic attacks almost immediately upon arriving at Queens, which have progressed to a level more severe than I have experienced in the past. This has even affected my ability to leave my room for food. This has led to a dangerous spiral of deteriorating physical health and depression including thoughts of self-harm. I am likely going to withdraw/defer the second semester so I can prioritize my health and build myself back to a stable condition. My overall academic performance has suffered greatly in the last month. Because of worsening health such as a lack of sleeping and eating, I began missing many classes. Anishinaabe was much harder to keep up with as the content was primarily taught through in-person activities. My actions led me to regularly feel physically ill which made it difficult to focus on large assignments. I have made a large effort to catch up in the last week. However, at the time of writing this, I believe only three of my courses will be achievable. I recently aged out of my previous support system. I began having sessions approximately a month ago with a therapist who will be providing me with a letter documenting the above. As soon as I obtain the letter, I will forward it to the appropriate place. In the meantime, I am hoping to concentrate on a fewer number of subjects as the semester is almost over and do as little harm as possible to my academic standing and my future.
