Petition Letter: Dear Associate Dean,
My name is Xiangyi Chen. I am appealing for the extension of the incomplete grade on the GPHY 370 and the late drop on GPHY 227 in the 2021 winter semester.
For a year, I was tortured by major depression. My mental state fluctuated, particularly aggravated in November as I was sent to the first aid center. My heart disease repeated meanwhile, as a result, I had to stop all work. When I feel good enough to continue my outstanding work, I contact the instructors in both courses. They permit me to complete the rest this semester.
I had accommodation for 227& 370 in the winter semester. Particularly for 227, I think I should explain why it appeared twice on my transcript. I enrolled in fall again as I intended to retake without bothering anyone else for my circumstance. I felt very awkward and ashamed for me to ask for accommodation. However, I never thought that my mental state worsened later so I had to stop for treatment. I contact both instructor and academic advisor about my situation. They suggest dropping the winter term and continue work on the fall term. I am sorry for this which results in trouble for everyone to deal with my situation. I would attach the supporting documents for both semesters to prove my circumstance.
